Excelera DCE is a Direct Contracting Entity (DCE) that contracts with the Centers for Medicare and Medicaid Services (CMS) to serve and lower costs for Medicare Fee-For-Service patients.

Rather than volume-based, reactive, acute care, we focus on prevention, wellness, and the management of chronic disease for Medicare beneficiaries across the nation. Our data-driven approach looks at populations as a whole, identifying the most vulnerable individuals. The Excelera DCE model shares savings with our physician partners. These savings are realized NOT by restricting access to care, but by reducing avoidable high-cost events that drive healthcare expenditures.

 We have a unique financial model that is potentially more lucrative for Primary Care Physicians than any other DCE offering today.

The Excelera DCE model rewards physicians and associated providers who deliver care to Medicare FFS beneficiaries. We also offer substantial prospective payments; something no other DCE, of which we are aware, offers.
